- To be used by State Library of Tasmania patrons. An information and reference service is provided by experienced professional staff from the State Reference Service. Staff are on hand to help and advise you in using the catalogues, reference collections and facilities.

- If the State Library does not have an item you want or information on a specific subject, fill in the form and we may be able to purchase available material where possible.
- Izone Stack Retrieval Form
- Many of the State Reference Service and Tasmanian Archive and Heritage Office (TAHO) books, serials and other materials are housed in a separate area known as the Stack. These items have to be retrieved by library staff. To avoid waiting for this service, you may use our retrieval form . All we need is one hour's notice and we will have your stack request waiting for you when you arrive.

- If you have changed your email address lately, please complete the following form so we can update our records. You can also use this form to notify library staff that you would like to receive your hold notifications via email.

- Use this form to notify library staff that you have lost your librarycard.
- Website Filtering
- It is possible that a website can be wrongly classified by our filtering software. If you would like to submit a filtered website for re-classification, please follow the steps detailed in the link above.

- Recorded Book Service
- The Recorded Book Service helps people with a print disability to access books and information on cassette and CD. The books in this collection have copyright permission from publishers and authors for use by people with a print disability only. Clients must have this Copyright declaration form signed by an authorised practitioner such as, a GP, social worker or disability service provider, in order to be eligible for this service.
- Relief Register
- The State Library maintains a listing of librarians and library technicians available to undertake relief work. This may include cover for sick leave (sometimes at short notice), holiday relief, or special contracts for specified project work. Anyone wishing to be added to the relief register should complete this form.
